ISO 7101 · Management systems for quality in healthcare organisations
ISO 7101 certification for hospitals and clinics: care quality you can show, not just claim.
ISO 7101 turns quality of care from a set of good intentions into a managed system: leadership that owns it, risks to patients and staff identified early, services delivered consistently and results reviewed on evidence. Sterling works with your clinical and operational teams to build that system, prepare it for an independent certification audit and keep it working afterwards. We ensure successful certification for our clients.
- Fixed scope, one proposal: consulting, Effivity software, Sterling Academy access and internal auditor training, as applicable to your standard, in one proposal
- A team of expert and experienced quality management consultants, with management system consulting across sectors and countries since 2003
- The Sterling Promise: 100% on-time completion, 100% success, 8,100+ client engagements. Sterling does not certify; an independent certification body audits and decides

Sterling client experienceWhat it is
What is ISO 7101 and what is it used for?
ISO 7101 is the first international management system standard written specifically for quality in healthcare organisations. It was developed by ISO/TC 304, the ISO committee for healthcare organisation management, and sets requirements for a systematic approach to sustainable, high-quality care. ISO describes it as applicable to providers from small clinics to large hospitals.
The standard requires top management to lead a culture of quality and to build care around people: respect, compassion, co-production, equity and dignity sit alongside risk identification, patient and workforce safety, documented service delivery, performance monitoring and improvement. In practice it gives a hospital group, a day-surgery centre or a diagnostic clinic one framework for decisions that are often spread across committees, departments and spreadsheets.
ISO lists ISO 7101 as a Type A management system standard on the harmonized structure, which means it contains requirements that can be certified and shares its layout with ISO 9001, ISO/IEC 27001 and ISO 45001.
Current edition. ISO 7101:2023, edition 1, published in October 2023. The ISO catalogue lists no amendment. Organisations implementing now build to this edition.
Who it is for
Who needs ISO 7101 certification?
ISO 7101 fits any organisation that delivers or manages health services: hospitals, clinics, diagnostic centres, rehabilitation and long-term care providers, and groups running several of them. Healthcare organisations usually come to it for one of four reasons:
- Patients, payers or corporate customers want assurance. Insurers, employers and medical-travel partners may ask how quality is governed. An audited system answers that question with evidence rather than a brochure.
- A tender or partnership asks for it. Public procurement, outsourcing contracts and hospital management agreements may name ISO 7101 or an equivalent quality system. Confirm the scope, sites and certificate wording they expect before you start.
- A regulator or health authority is raising the bar. Licensing inspections and national quality programmes often look for governance, incident learning and measured outcomes. ISO 7101 organises that work into one system that stands up to scrutiny.
- A group needs one approach across its facilities. Several hospitals or clinics under common ownership, each with its own habits, need shared quality processes with clear local accountability.
Requirements
What are the main ISO 7101 requirements?
ISO 7101 follows the harmonized structure used by ISO management system standards, with requirements in clauses 4 to 10. The headings below are those of the harmonized structure; the notes describe how each heading plays out in a healthcare setting. The detailed ISO 7101 requirements under each heading are checked against the standard itself during the gap analysis.
- Clause 4 Context of the organisation: the services, patient groups, interested parties and boundaries the quality system covers.
- Clause 5 Leadership: top management accountable for care quality, with a policy, roles and a culture that puts people at the centre.
- Clause 6 Planning: risks and opportunities for patients, staff and services, and quality objectives that can be measured.
- Clause 7 Support: competent and aware staff, communication, resources and controlled documented information.
- Clause 8 Operation: planning and control of care and support services, including outsourced and contracted work.
- Clause 9 Performance evaluation: monitoring and measurement, patient experience, internal audit and management review.
- Clause 10 Improvement: acting on incidents and nonconformities, correcting causes and improving continually.
What does ISO 7101 evidence look like in a hospital or clinic?
Minutes of a quality committee that took decisions, a risk register that names patient-safety and staff-safety risks with owners, credentialing and training records, care pathways people follow, incident reports with investigations and closed actions, patient feedback that reached management, and indicators reviewed on a schedule. We help you use the records clinical teams already keep, and they sit in Effivity so the auditor sees a system in use.
Certification process
How do you get ISO 7101 certified?
The route has two halves: your organisation implements the system, then an independent certification body audits it. Sterling supports the first half and prepares you for the second; the certification body alone decides whether to certify.
- 01Agree the scope of the certificate.Decide which facilities, departments, clinical and support services the certificate should cover, and what patients, payers or tenders expect it to say.
- 02Gap analysis: compare current practice with ISO 7101.Governance, risk, clinical and support processes, records and indicators are reviewed; what works is kept, and actions, owners and dates are agreed for the rest.
- 03Implement across wards, clinics and support functions.Controls are put into daily use, staff are trained and the system produces records over a period long enough to show it operates.
- 04Internal audit and management review.Trained internal auditors test the system, top management reviews the results, and findings are closed with evidence before the external audit.
- 05Stage 1, Stage 2 and the certification decision. The certification body holds a Stage 1 readiness review and a Stage 2 audit, then decides; certification runs on a three-year cycle with surveillance audits at least once each calendar year.
Check the route before you sign. Ask which certification body will audit, whether it is accredited for management system certification under ISO/IEC 17021-1, and whether that accreditation covers ISO 7101 in your country. Accreditation recognition is the Global ACI arrangement; verify the body and its scope on the accreditation body's own register, not on the certification body's letterhead.
Cost
How much does ISO 7101 certification cost?
The cost of ISO 7101 is set by the shape of your organisation, not by a price list. A single outpatient clinic with a working quality committee needs far less than a multi-hospital group building a common system from scratch. Sterling puts every component in writing under one all-inclusive proposal.
| Cost component | What affects it |
|---|---|
| Consulting and implementation | Number of facilities and specialties, the maturity of existing quality and patient-safety processes, and whether you choose Design™ or Drive™ |
| Independent certification | The certification body's audit days, which depend on scope, headcount and sites, plus travel and yearly surveillance |
| Internal resources | Time from clinical leads, nursing, quality staff and management, and any process changes the gap analysis calls for |
| Software and learning | Effivity and Sterling Academy access are part of the engagement, with users and courses confirmed in the proposal |
| Continued support | Internal audits, indicator reviews and system upkeep under Sustain™ |
What moves the price
- Number of facilities, beds and clinical departments in scope
- Headcount across clinical and non-clinical staff
- Whether an ISO 9001 system or a hospital accreditation programme already exists
- Outsourced services such as laboratory, imaging or catering
- The target date and the time your teams can give
What a Sterling proposal always shows
- Scope, deliverables, assumptions and exclusions, written down
- Certification-body fees listed on their own line
- Effivity, training and Academy access itemised at no additional cost
- Payments linked to delivered milestones
- One all-inclusive proposal. No surprises, ever
Timeline
How long does ISO 7101 certification take?
Healthcare adds its own pace-setters: clinicians' availability, committee cycles, shift patterns and the need for indicators to run long enough to show a trend. The system has to operate and produce evidence before the certification body arrives, so the plan is measured in months: as an estimate, often six to twelve for a first system. We confirm the realistic range after the gap analysis and track milestones in Effivity.
| Phase | What determines the pace |
|---|---|
| Scope and gap analysis | Access to department heads, facilities, policies and current records |
| Build and implementation | How much governance, risk and pathway work is new, approval routes and staff training |
| Evidence and review | Indicators and incident learning running long enough to show results, then internal audit and management review |
| Certification assessment | The certification body's schedule for Stage 1 and Stage 2 and the time to close any findings |
Tell us any tender or contract date on day one, and we will say plainly what is achievable.

What this means at the audit
An ISO 7101 auditor will want the risk register, recent incident investigations, the indicator trend, the internal audit results and the last management review. In Effivity each one opens on screen with dates, owners and evidence, instead of a file assembled the week before. Sustain™ continues in the same workspace, so surveillance visits meet the same live system.

FAQs
Frequently asked questions about ISO 7101
What is the difference between ISO 7101 and ISO 9001?
ISO 9001 is a general quality management standard for any sector, while ISO 7101 is written for healthcare organisations. Both use the harmonized structure, so an ISO 9001 system is a strong starting point, but ISO 7101 adds a healthcare focus on people-centred care, patient and workforce safety and quality culture led from the top. A gap analysis shows exactly what an existing ISO 9001 system still needs.
Is ISO 7101 certification mandatory for hospitals?
Not as a general rule. ISO itself notes that certification to its management system standards is not a requirement, although a contract, tender or health authority can make it one. Many providers adopt ISO 7101 simply to run quality in a more disciplined way.
Does ISO 7101 replace hospital accreditation programmes?
No. Hospital accreditation programmes are run by their own bodies against their own standards, while ISO 7101 certification is a management system audit by a certification body. The two can run side by side; whether one programme gives credit for the other is for that programme owner to confirm.
Can a small clinic get ISO 7101 certified?
Yes. ISO describes the standard as applicable to providers from small clinics to large hospitals. A small clinic needs a proportionate system: clear roles, a working risk register, a few meaningful indicators and a regular review, not a hospital-sized manual.
Can ISO 7101 be combined with ISO/IEC 27001 or ISO 45001?
Yes. All three share the harmonized structure, so document control, internal audit and management review can serve one integrated system covering care quality, information security and staff safety. Each standard's own requirements still have to be met and audited.
