What it is

What is CAP accreditation and what is it used for?

CAP accreditation is granted by the College of American Pathologists through its Laboratory Accreditation Program. The programme rests on the CAP's Standards for Accreditation and on detailed checklists that set out what inspectors will look for, from test method validation and verification to proficiency testing, quality control, personnel and records. Laboratories are inspected by peers, people who work in laboratory medicine themselves, rather than by a certification auditor.

In the United States the CAP is approved by the Centers for Medicare & Medicaid Services as an accreditation organisation, and its checklists track the federal CLIA rules. Outside the United States, laboratories choose CAP accreditation because hospitals, research sponsors, insurers and referring clinicians recognise it as a demanding benchmark. The CAP runs a dedicated route for laboratories outside the USA.

Current edition. The CAP revises its checklists and releases a new edition every year. Recent editions have added requirements on digital pathology and patient self-collection, and the 2024 edition carried the CLIA final rule changes for US laboratories. Your preparation is set against the edition the CAP applies at your inspection, and every annual release is reviewed for changes that touch your disciplines. The CAP also runs a separate CAP 15189 programme; if ISO 15189 is what your customers ask for, start with our ISO 15189 accreditation page.

Who it is for

Who needs CAP accreditation?

The Laboratory Accreditation Program is built for medical laboratories, and the CAP offers a system inspection option for laboratory networks. Laboratories come to Sterling for CAP readiness from four starting points:

  • A hospital, sponsor or referral partner expects it. Research sponsors, international patients and referring hospitals may ask for CAP accreditation by name. Confirm exactly which programme and which disciplines they mean before the work begins.
  • A tender or payer contract lists it. Some procurement documents for diagnostic services name CAP accreditation as a qualifying condition or a scoring criterion.
  • A regulator or health authority recognises it. In the United States, CAP accreditation is one way to meet CLIA oversight; elsewhere, some authorities and insurers give weight to it. Check what your own authority accepts.
  • A group standardises its laboratories. A laboratory chain or hospital group wants every site working to the same checklists, with one quality approach and one set of records.

Accreditation reflects how the laboratory actually runs on inspection day and every day after. The value lies in the practice behind it, not the certificate on the wall.

Requirements

What are the main CAP accreditation requirements?

The CAP publishes its Standards for Accreditation and a set of checklists; the inspection tests your laboratory against the checklist requirements that apply to your disciplines. The map below is our plain-language grouping, not the CAP's own checklist titles. Your readiness work follows the current checklist text, requirement by requirement.

  • Standards for Accreditation: the overarching principles that the checklists put into detail.
  • Test method validation and verification: evidence that each method performs as intended in your laboratory before patient results are released.
  • Proficiency testing: enrolment in suitable external programmes, review of every result and action on unacceptable ones.
  • Quality control and quality management: routine control of examinations, documented review and a quality programme that finds and fixes problems.
  • Patient safety and specimen handling: from collection, including patient self-collection where offered, to identification, reporting and retention.
  • Personnel and competence: qualifications, training and ongoing competence assessment for everyone who performs testing.
  • Discipline-specific practice: requirements for each area you test in, including digital pathology where it is used.
  • Self-inspection: the laboratory inspects itself against the checklists and records the outcome on the CAP's verification form.

What does useful CAP inspection evidence look like?

Validation and verification studies you can explain, proficiency testing results with signed reviews and corrective action, quality control records with trends someone actually looked at, competence assessments on file for each person and method, and a self-inspection that found real issues and closed them. We help your team assemble exactly this evidence, and keep the quality records in Effivity so they are current whenever an inspector asks.

Accreditation process

How do you get CAP accreditation?

Build a laboratory that already meets the checklists, then let the CAP inspect it. Sterling prepares the laboratory; the peer inspection and the accreditation decision belong to the CAP.

  1. 01
Confirm the programme and the scope.Decide on the Laboratory Accreditation Program or another CAP programme, and agree the disciplines, sites and tests that the application will cover.
  2. 02
Gap analysis: compare the laboratory with the current checklists.Work discipline by discipline, keep what already meets the requirement and agree actions, owners and dates for the rest.
  3. 03
Build the evidence.Complete validation and verification studies, put proficiency testing and quality control reviews on a routine footing, finish competence assessments and bring procedures and records up to date.
  4. 04
Run a rigorous self-inspection.Inspect the laboratory against every applicable checklist requirement, as a peer inspector would, and close each finding with evidence.
  5. 05
Apply, host the peer inspection and respond.Submit the application to the CAP, host its inspection team, correct any deficiencies within the time the CAP sets and await its decision; then keep the laboratory inspection-ready on the cycle the CAP sets.

Check the route before you sign anything. Only the College of American Pathologists grants CAP accreditation; no consultant or certification body can issue it on the CAP's behalf. You can confirm any laboratory's status in the CAP's Accredited Laboratory and Biorepository Directory on cap.org.

Cost

How much does CAP accreditation readiness cost?

Readiness costs depend far more on the state of your records than on the size of your building. A single-discipline laboratory with strong validation files needs a lighter programme than a multi-site service still building its quality records. Every Sterling proposal shows each component, and nothing is added later.

Cost componentWhat affects it
Consulting and readinessDisciplines and sites in scope, the gaps found in the gap analysis, and whether you choose Design™ or Drive™
CAP programme feesSet by the CAP for its programme, your laboratory and your inspection; paid to the CAP, not to Sterling
Technical workValidation and verification studies, proficiency testing enrolment, calibration and any equipment or environment changes
Internal resourcesTime from the laboratory director, section leads and testing staff for records, competence assessment and the self-inspection
Software and learningIncluded in a Sterling engagement: Effivity for the quality records and Sterling Academy access for your people

What moves the price

  • The number of disciplines, tests and sites in scope
  • How many methods lack validation or verification records
  • Proficiency testing history and how it was reviewed
  • The target date and your team's available time

What a Sterling proposal always shows

  • Deliverables, assumptions and exclusions in writing
  • CAP fees kept separate from our fee
  • Milestones linked to deliverables
  • One all-inclusive proposal for our work

Timeline

How long does CAP accreditation take?

Three things set the pace: how many methods need validation or verification work, how much proficiency testing and quality control history you can show, and when the CAP can schedule your inspection. As an estimate, a first accreditation typically takes nine to fifteen months from gap analysis to decision; a laboratory with strong records moves faster.

PhaseWhat sets the pace
Scope and gap analysisAccess to section leads, method files and quality records
Evidence buildValidation and verification studies, competence assessments and procedure updates
Operating historyProficiency testing events and quality control data accumulating under normal work

FAQs

Frequently asked questions about CAP accreditation

Is CAP accreditation the same as ISO 15189 accreditation?

No. The Laboratory Accreditation Program is the CAP's own programme, built on its checklists. The CAP also runs a separate CAP 15189 programme against the international standard; a laboratory can hold one, the other or both, depending on what its customers and regulators ask for.

Can a laboratory outside the United States get CAP accreditation?

Yes. The CAP accredits laboratories outside the USA and publishes guidance on what they should expect. Local licensing and health authority rules still apply alongside it, and Sterling works through both with your team.

What is a CAP self-inspection?

It is the laboratory's own inspection of itself against the applicable checklists, recorded on the CAP's verification form. Treated seriously, it is the best rehearsal for the peer inspection; treated as paperwork, it hides the gaps inspectors later find.

How often do the CAP checklists change?

Every year. The CAP releases a new checklist edition annually, and changes can add, revise or retire requirements. Accredited laboratories need a routine for reviewing each release and updating procedures before their next inspection.

Does Sterling accredit laboratories or carry out CAP inspections?

No. Sterling prepares laboratories. Only the College of American Pathologists grants CAP accreditation, and its peer inspectors carry out the inspection.
