Sterling client experienceWhat it is
What is ICH GCP (E6) and what is it used for?
ICH E6 is the Guideline for Good Clinical Practice issued by the International Council for Harmonisation, the body through which medicines regulators and industry agree common technical guidance. Good clinical practice is the ethical and scientific quality expectation for trials that involve human participants: how a study is designed, approved, conducted, monitored, recorded and reported, so that the people who take part are protected and the data submitted to regulators is reliable.
In practice GCP is a quality discipline shared between several parties. A sponsor decides what the trial is for and how it will be overseen; investigators and their sites carry it out with participants; service providers take on delegated tasks. Each needs procedures, trained people, controlled records and a way to find and fix problems before they touch a participant or a dataset.
Current edition. ICH E6(R3), the third revision of the guideline. Its principles and Annex 1 were adopted at Step 4 of the ICH process on 6 January 2025, and Annex 2 has since been adopted and published. ICH adoption is not the same as local effect: each regulator implements the guideline within its own framework and on its own timetable, as the European Medicines Agency and the US FDA have done in their own guidance documents. Confirm the date that applies to your studies with the regulator concerned.
Who it is for
Who needs ICH GCP compliance?
Any organisation that sponsors, conducts or supports clinical trials of medicines is expected to work to GCP, whether it runs one investigator-initiated study or a portfolio across several countries. The organisations that come to us usually start from one of four places:
- A sponsor or partner is qualifying you. Pharmaceutical companies and contract research organisations audit the sites, laboratories and vendors they use, and expect a working GCP system before they place a study.
- A tender or grant sets GCP as a condition. Funders and public programmes ask for evidence that trial conduct and data handling meet recognised expectations; the proposal needs more than a training log.
- A regulatory inspection is planned or possible. A marketing application, a notified inspection or a routine programme puts your trial records in front of an inspector, and the system has to stand on its own.
- A group company or new market expects one approach. Sites and affiliates in different countries follow different procedures, and E6(R3) is the moment to bring them onto one system.
Requirements
What are the main ICH E6(R3) requirements?
E6(R3) is organised in three parts. The list below is a plain-language map of that structure; during the programme your procedures and records are checked against the guideline text itself, and against any national rules your regulator applies alongside it.
- Principles of GCP: the overarching expectations that frame every trial, centred on protecting the people who take part and on producing results that can be relied on.
- Annex 1: the detailed expectations that put the principles into practice for each party involved in a trial, read according to the role your organisation plays.
- Annex 2: additional considerations published as a separate annex, which a sponsor reads with the principles and Annex 1 when a trial's design calls for it.
Whichever parts apply, the test is the same: can each party show who was responsible for what, that decisions were proportionate to the risks to participants and data, and that problems were found, investigated and corrected?
What does useful GCP evidence look like?
Controlled SOPs matched to your role in the trial, a delegation and training record for every person with trial duties, vendor qualification and oversight files, risk assessments that shaped the monitoring approach, deviation and CAPA records closed with root cause, internal audit reports and trial records complete enough to reconstruct events. We keep them in Effivity, so an inspector sees a system in use rather than a file assembled for the visit.
Getting inspection-ready
How do you achieve ICH GCP compliance?
There is no certification audit at the end of a GCP programme. Readiness is shown in sponsor audits and, where it happens, a regulatory inspection of your studies. Sterling helps build and run the system; the regulator alone inspects and reaches conclusions.
- 01Define your role and the studies in scope.Confirm whether you act as sponsor, site, contract research organisation or vendor, the countries and regulators involved, and the studies the system must cover.
- 02Gap analysis: compare current practice with E6(R3).The gap analysis reads your SOPs, oversight arrangements and trial records against the guideline, keeps what works and sets actions, owners and dates.
- 03Build the clinical quality system.Write or revise procedures, set up risk-based oversight of studies and vendors, and train everyone with trial duties.
- 04Audit and correct.Internal audits of processes, studies and suppliers, with deviations and CAPA closed on evidence before an external party looks.
- 05Prepare for inspection and keep the system running.Mock inspections, front-room and back-room preparation and ongoing audits, so sponsor audits and regulatory inspections meet a system in daily use.
Check the route before you sign. No organisation can be certified to ICH GCP, so treat any offer of a "GCP certificate" for your company as a warning sign. Inspection outcomes come from the regulator, through its own channels; confirm them there, not from a provider's paperwork. How to choose a credible GCP consultant
Cost
How much does ICH GCP compliance cost?
A single site refreshing its SOPs and a sponsor overseeing vendors in several countries face different tasks, so there is no sensible list price. What we commit to is an all-inclusive proposal with every component shown.
| Cost component | What affects it |
|---|---|
| Consulting and implementation | Your role in trials, the maturity of current procedures, the number of studies and sites, and whether you choose Design™ or Drive™ support |
| Audits and inspection preparation | Internal and vendor audits required, mock inspections, travel to sites, and the scale of any open findings from past audits |
| Internal resources | Time from study teams, quality staff and management, procedure reviews, training hours and corrective work |
| Software and learning | Included in a Sterling engagement: Effivity for the quality system and Sterling Academy access for your people, with users and access period confirmed in the proposal |
| Continued support | Periodic audits, procedure upkeep, refresher training and support as studies start and close under Sustain™ |
What moves the price
- Your role: sponsor, site, contract research organisation or vendor
- The number of active studies, sites and countries in scope
- How many vendors you delegate trial tasks to and must oversee
- Whether procedures exist today and how current they are
- Inspection or sponsor-audit deadlines already in the calendar
What a Sterling proposal always shows
- Deliverables, responsibilities, assumptions and exclusions, in writing
- Third-party costs such as translation or site travel identified separately
- Effivity, training and Academy access itemised at no additional cost
- Payment milestones linked to deliverables
- One all-inclusive proposal. No surprises, ever
Timeline
How long does ICH GCP compliance take?
Timing depends on your role, the studies already running, the state of your procedures and how much time study teams can give while trials continue. As an estimate, building or re-basing a GCP quality system to the point of inspection readiness takes four to nine months; live studies usually set the pace, because changes have to be introduced without disrupting participants.
| Phase | What determines the pace |
|---|---|
| Scope and gap analysis | Access to SOPs, trial files, vendor contracts and the people who run studies |
| Procedures and training | The number of procedures to revise, review and approval cycles, and training across sites |
| Operation and audit | Procedures in use long enough to leave records, followed by internal and vendor audits |
| Inspection readiness | Mock inspection findings, CAPA closure and any sponsor or regulator dates already fixed |
If a sponsor audit or inspection is already scheduled, tell us at the start; we will say what can be ready and what cannot.

Effivity · ISO and GRC software, included in every engagement
How Effivity helps you fulfil, automate and simplify the requirements of ICH E6(R3).
Every Sterling GCP engagement includes Effivity, the ISO and GRC automation platform built by the Sterling group and used in 50+ countries. Your consultant configures a workspace within the agreed scope for controlled SOPs, risk registers, internal and vendor audits, deviations and CAPA, training records and management review, so the quality system runs alongside your studies and an inspector reviews live records. Sterling consulting clients receive their Effivity subscription included with the engagement, at no additional cost.

Scope, roles and responsibilities
Your role in each study, the responsibilities you hold and the tasks you delegate are recorded in one place and reviewed when a study starts or a contract changes.
Quality objectives and management review
Objectives, trial quality indicators and review decisions sit with their owners, and the next management review draws on them without a separate spreadsheet.
Risks to participants and data
A register of study and process risks, with owners, controls and review dates, feeding the monitoring and audit plans.
Competence, training and controlled SOPs
Training records for every person with trial duties, linked to Sterling Academy; SOPs with versions, approvals and read-and-understood records.
Supplier evaluation and oversight
Qualification, contracts, performance reviews and audits of contract research organisations, laboratories and technology vendors, each with its record kept as the work happens.
Internal audit programme
Process, study and vendor audits planned, reported and followed up, with findings traced to closure.
Deviations, nonconformities and CAPA
Raised, investigated for root cause and closed with evidence that the action worked; repeat issues visible before an inspector finds them.
Your own forms and reports
Forms, workflows and reports shaped to your trial processes without code, with regulatory requirements linked to the procedures they affect.
What this means at an inspection
An inspector or sponsor auditor typically asks for SOPs in force on given dates, training records for named roles, vendor oversight, the audit programme and open CAPA. In Effivity each of these is a screen with dates, owners and evidence attached. Between inspections Sustain™ runs in the same workspace, so the system never goes quiet.

The Sterling Promise
What our ICH GCP success guarantee means
We stay with you until your agreed ICH GCP compliance outcome is achieved, supporting implementation, preparation and the resolution of audit findings. Clear explanations, practical delivery, included software and employee learning turn the investment into lasting capability rather than a binder on a shelf.

Regulators inspect and reach their own conclusions. Our commitment concerns the agreed compliance support, not a promise of every future sponsor award or inspection outcome.
FAQs
Frequently asked questions about ICH GCP
Can a company get ICH GCP certified?
No. ICH E6 is a guideline, and ICH does not certify organisations; nor does any accredited scheme certify companies against it. What you can show is a working GCP quality system, clean sponsor audits and, where it happens, the outcome of a regulatory inspection.
Is ICH E6(R3) legally binding?
It depends on the country. ICH guidelines take effect through each regulator's own framework, so their standing varies with local law and the regulator's texts. Your legal and regulatory advisers confirm the position for each country; we build the system that meets it.
Does ICH GCP apply to CROs, laboratories and software vendors?
Yes, where they carry out trial-related work. Their obligations come through contracts with the sponsor and through regulators' expectations, so vendors are regularly audited by sponsors and can be drawn into inspections.
Does a GCP training certificate prove our organisation is compliant?
No. An individual's course certificate shows that one person completed training. Compliance is shown by procedures, oversight and records across your studies, of which training is only one part.
