Ideally while the design is still being shaped, because design controls, risk files and supplier records are far easier to build as you go than to reconstruct later. A start-up should start once it plans to place a device or software on the market, or when a partner asks for the system.
Does ISO 13485 itself give a medical device market access?
No. It certifies a quality management system, not a product. Market access follows the device regulation route set out on the country page. ISO 13485 supports that route and is often asked for by distributors and hospital buyers, but your regulatory adviser should confirm what your device needs.
